When meeting with your surgical coordinator, please ensure that you have provided the correct phone number and fax number for your primary care doctor or cardiologist. If your doctor’s office has not received the request, please verify their phone and fax numbers, then contact your surgical coordinator directly. Our team will then resend the request.
As a reminder, you have been provided with a copy of all medical clearance requirements in your surgery packet.
