Pre-Registration Process:

Thank you for choosing Fort Lauderdale Eye Institute for your eye care needs.  

We no longer use paper in-take/check-in forms. Rather, New and established patients will go through an electronic patient registration process. We highly recommend completing this process before you come to your appointment, as this will expedite your visit. However, if you are not able to complete the pre-registration before your visit, you will be able to pre-register in the office on an electronic tablet.  

What to expect:

A few days before your first visit as a new patient, you will receive either an email or a text message (or both if you gave us that information when you made your appointment).

The “email” will state on the subject line:

EPF- Fort Lauderdale Eye Institute <registration@phreesia-mail.com>

The email will ask you to check-in online, and will have an orange tab directing you to “CLICK HERE TO START”. Follow the instructions and you should be able to pre-register before your appointment.

The “Text Message” will originate from a 5 digit number:

Fort Lauderdale Eye (954) 741-5555,  Appt: your first name and day and time of appointment and a link to be able to check -in online. Press the link and you can pre-register on line, using our Phreesia system.

Registering online, ahead of time, will save you time once you get to our office, so we highly recommend it.  However, if you are not able to register ahead of time, please arrive 15 minutes earlier than your appointment, so you can complete the registration process in the office.

If you are not able to pre-register the first time you receive communication, you will continue to receive messages to your email and via text, daily until you complete the registration or until the day of your appointment.
