Hi, I’m Dr. Aliza Epstein, and I’m an oculoplastic surgeon at the Fort Lauderdale Eye Institute. In this video, I’m going to talk about the lower blepharoplasty procedure.

So, what is a lower blepharoplasty?

When patients have cosmetic concerns about the appearance of their lower eyelids—whether it’s fat, bulging, under-eye bags, or skin texture—these issues can be addressed with a cosmetic procedure called a lower blepharoplasty.

There are many different techniques that I use when performing a lower blepharoplasty because every patient is unique. The surgical plan must be tailored to each patient’s anatomy.

For some patients, excess fat can be excised. More commonly, however, I reposition or transpose the fat into the cheek to fill the hollow beneath the puffy area. This technique is called a fat transposition lower blepharoplasty.

In addition to addressing volume underneath the eyes with fat repositioning, we also address the skin and muscle of the lower eyelid and cheek. In some cases, skin is excised. In others, the skin is tightened using CO₂ laser skin resurfacing or a chemical peel to tighten and brighten the skin at the same time.

The lower blepharoplasty procedure is performed in an outpatient surgical center—we do not perform it in the office. It is an excellent procedure for making the eyes look more youthful and rejuvenated, and for helping patients feel more confident in their appearance.
