These procedures are typically not covered by insurance and are based on your insurance policy’s criteria for medical necessity. Cosmetic procedures are intended to enhance appearance and are not meant to address a medical issue. Therefore, insurance companies categorize them as elective procedures.
Although most cosmetic procedures are performed in an outpatient or ambulatory setting, facility and anesthesia fees are based on the length of time needed to perform the cosmetic portion of the surgery.
